On initiative count 20 (losing initiative ties), the 

 takes a lair action to cause one of the following effects; the 

 can't use the same effect two rounds in a row:

 can take 1 legendary action, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The 

 regains spent legendary actions at the start of its turn.

Though the origins—or even the existence—of the mountainheart parasite have been lost to history, there are said to still be mountains which harbour them. As their name implies, they infest mountains, large hills, or even glaciers on occasion, and merge with them. Over the course of years, they can gain influence over massive areas of land, poisoning the creeks and leaving entire slopes lifeless.

While some may be driven away by the parasite's influence, others are attracted by the parasite's influence, though few ever grasp its full nature. As a result, a mountainheart parasite might inadvertently amass a fervent group of followers who feed and protect it. Occasionally these cults might even predate the parasite's presence, having worked towards its summoning for years.
